DESCRIPTION: Nurse Practitioner Large, primary care office Full-Time Exempt $90,000 - $130,000 annually Nurse Practitioner Benefits: Annual performance review, performance-based merit increase Additional supplemental compensation available CME allowance Malpractice insurance, licensing fees, registration, and DEA fees paid by the practice. Our Nurse Practitioners have ready access and ability to precept cases with our Physicians in a supportive environment. Nurse Practitioner Responsibilities : Provide and coordinate health care services to patients and their families Promote maintenance of health, prevention of illness and stability Provide care during acute and chronic phases of illness Guide and counsel individuals and families Referral patients to other health care providers as appropriate Record and update patient EMR in accordance with HIPAA and practice standards and procedures Nurse Practitioner Qualifications: Current, certified NYS Nurse Practitioner license Minimum of two (2) years’ experience in a family practice or Urgent Care setting is desired (not required) Previous experience working with Medent (EMR) is a plus Excellent working knowledge of chronic disease states and possess critical thinking skills Strong interpersonal and communication skills POSITION REQUIREMENTS: Nurse Practitioner Qualifications: Certified NYS Nurse Practitioner license credential. A minimum of two (2) years’ experience in a family practice or Urgent Care setting is desired (not required) Previous experience working with Medent (EMR) is a plus. One of our main goals at CNY Family Care, LLP is to provide all of our patients with continuous and comprehensive health care. As part of this care model, we ask all of our patients to select one personal physician. This PCP/Primary Care Physician will maintain an ongoing relationship with the patient and will then always be in charge of their care. They will oversee the care team that consists of nurses, Nurse Practitioners, Physician Assistants and Diabetic Educators who might interact with the patients when they visit the office. Our care team will work towards ensuring that the patients receive the appropriate age-related preventative testing and treatment as well as the latest evidence-based treatment (proven and documented) for their individual medical condition(s).
