As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what's best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. The PACE Home Care Certified Nursing Assistant (CNA) provides direct personal care and supportive services to participants in their homes, supporting independence, safety, and quality of life. This role focuses on delivering one-one-one care in a home setting, including assistance with activities of daily living, light housekeeping, meal preparation, and observation of participant condition.
Working under the supervision of a Registered Nurse (RN), the Home Care CNA plays a key role in identifying and reporting changes in participants' physical, mental, or environmental status to ensure timely intervention and continuity of care. The position requires the ability to work independently in the field while maintaining strong communication with the Interdisciplinary Team (IDT).
High school diploma or General Education Degree (GED) required. Must be at least 18 years of age.
